Following is the indicative list of information, which you generally provide, when you appointment with us  (a) your name, email address, phone number, adress (b) information you provide us when you contact us for help.

We may use the information you provide us to contact you from time to time to know when you visit our Hospital. We will ask you when we need more information that personally identifies you (personal information) or allows us to contact you.

We take your email address to give a copy of your document, when you submit our appointment form to see that what you share with us.
